Dr.Diary, Republic of Korea's leading chronic disease management platform, presented how it reconnected metabolic care that had been fragmented across prevention, diagnosis, treatment, and long-term management through integrated data and AI-powered coaching. The session examined Korea's chronic care gap (the care cascade) and introduced its "daily-life engagement" model, built around the app, continuous glucose monitoring (CGM), and commerce. It also demonstrated how the model had been scaled across consumers, clinics, enterprises, and public institutions (B2C, B2H, and B2B). The session concluded by exploring the model's relevance to chronic disease and population health management, as well as opportunities for international collaboration.
